Instructions
- Please carry all your previous medical records and reports.
- Please carry your first morning urine and stool samples in plastic that have been sent to you. Please collect the urine after discarding the stream in the first few and the last few seconds.
- Please come fasting for 12 hours overnight for the check up. Kindly avoid alcohol and food items with excess fat like cheese, deep fried preparations on the day prior to your check up.
- Please do not consume tea/ coffee/ biscuits on the morning of the programme. You should drink 1 Litre of plain water before you arrive at the hospital. You should avoid passing the urine after collection of morning sample till your ultrasound test is over.
- If you are on medication, kindly consume them as prescribed. Do not skip your blood pressure medicines on the day of check up.
- If you are Diabetic, please do not consume for morning dose of Insulin or morning tablets on the day of check up. Please carry them along with you and take them at the time of breakfast, which will be served to you in hospital.
- Please do not wear any jewellery on the day of your check up.
- Do not smoke on the day of your check up, until all investigations are completed.
- If you use contact lenses, please do not wear them on the day of your check up. Wear glasses instead.
- Please do not apply cream, oil or talcum powder when you come to the hospital.
- On arrival you would be given a comfortable tracksuit that you will be required to wear in the hospital.
- Please wear/carry your comfortable sport shoes/sneakers.
- Please be prepared to spend 6-8 hours in hospital on the day of check up. Do carry some reading material with you. Televisions and internet kiosks are available in the Programme Lounge.
- You will be served breakfast during the programme.
- A follow up meeting with the Internal Medical Consultant will be scheduled at mutually convenient time. Your reports with be discussed and necessary follow up advice will be given.
Special Note for Diabetics
- Please inform your Patient Care Associate about your condition at the time of registration.
- Eye Drops may be administered to you for detailed examination. As a result you may not be able to see things clearly for few hours afterwards.

The LASIK details are as follows Technology
Artemis
Health Institute is using the : "VISX Star IR4, which is the most
advanced Ecimer Laser system from Abbott Medical optics" from AMO-ABOTT.
The procedure we are performing is Advanced CustomVue LASIK for myopia,
astigmatism and hypermetropia. This can correct refractive errors and
thus eliminate the need for spectacles and contact lenses. The use of
customized or wave front technology ensures correction of even the finer
irregularities in the eye resulting in a better quality of vision.

The Medical treatment and tourism Itinerary - 6 Days
Day 1 - New Delhi - Gurgaon Welcome to Gandhi International Airport, New Delhi, India. We will be there to meet you at the airport and transfer you to your hotel. Upon arrival at the hotel in Gurgaon, we greet you with a warm traditional Indian welcome. After check in, we will guide you to your room for a well-deserved rest after your long journey. You will then proceed to your LASIK work-up and examination in the ophthalmology OPD to confirm your good condition for the LASIK procedure. You will stay overnight at the hotel. Day 2 - Gurgaon After a delicious breakfast, we will take you to your LASIK surgery. The approximate duration is 20 minutes. You will be kept in the OPD area for one hour after the procedure. Day 3 - Gurgaon After breakfast, we will take you for your follow up examination. After the examination, you will rest to allow for proper healing. Day 4 - Gurgaon - Agra After breakfast, we will proceed to Agra via a taxi ride of approximately five hours, where we check in to the JAYPEE PALACE HOTEL. Later we will visit one of the magnificent, Seven Wonders of the World! THE TAJ MAHALIf time permits, we will also visit the Agra Fort. The majestic red sandstone fort of Agra stands on the banks of the river Yamuna. The construction was started by Emperor Akbar in 1566. At the Diwan-I-Am, (Hall of Public Audiences), a colonnaded hall of red sandstone with a throne alcove of inlaid marble at the back, the Emperor heard public petitions. At the Diwan-I-Khas, (Hall of Private Audiences), where marble pavilions with floral inlays lend an ethereal atmosphere, the Emperor sat on his gem-studded Peacock Throne and met foreign ambassadors and rulers of friendly kingdoms. Your evening will be free to explore Agra's rich heritage of handicrafts in its market. Afterward, you will return to the Jaypee Palace Hotel for your overnight stay in Agra. Day 5- Agra - Gurgaon After breakfast, we return to Gurgaon via taxi & do the follow up visit . Later we will visit the Ambience Mall. The Ambience Shopping Mall is a complete new experience for shoppers in Gurgaon. It's the largest shopping mall in India. Shopping at the Ambience Mall is an event in itself as it would require an entire day to see everything it has to offer. Stores like Debenhams and Reliance Timeout would take most of your shopping time and you would enjoy every moment of your day. The international brands that you will see in the mall are exclusive, such as Jumbo Electronics, Sony World, Debenhams, Reebok and more! Ambi Mall (as it is fondly known) covers one kilometre area on each of its three floors. This means it is a three kilometre area shopping mall. The real specialty of the stores in Ambience mall is the huge space they occupy. Almost all of the stores within this mall are enormous in total covered area with their fabulous interiors. Day 6 - Gurgaon Today we check out from the hotel and transfer you to your onward destination drop at the airport. |

- 10 days before departure - 100% of holiday cost
- In the event of a no show for the holiday - 100 % of holiday cost
- No refund either in part or in full will be made for any unused portion of the services provided in the package.
